Milrinone was introduced as an agent which, compared to dobutamine, cause reduced left and right heart filling pressures due to its greater reduction in vascular resistance, and thus superior to dobutamine in treatment of low cardiac output syndrome following cardiac surgery [30, 31].

What is the difference between dobutamine and milrinone?

Milrinone is a phosphodiesterase 3 inhibitor that increases cardiac inotropy, lusitropy, and peripheral vasodilatation. In contrast, dobutamine is a synthetic catecholamine that acts as a β₁- and β₂-receptor agonist and improves blood pressure by increasing cardiac output.

When do you take dobutamine over milrinone?

Inotropic therapy with milrinone or dobutamine can be used in chronic heart failure patients as a bridge to recovery from an acute hemodynamically compromised state and in advanced stage heart failure patients awaiting advanced heart failure therapies, such as mechanical circulatory support and heart transplant.

What are the benefits of milrinone?

Milrinone is a phosphodiesterase-3 inhibitor that inhibits the degradation of cyclic adenosine monophosphate (cAMP). By increasing the concentration of cAMP, milrinone enhances myocardial contractility, promotes myocardial relaxation, and decreases vascular tone in the systemic and pulmonary circulation.

Why dobutamine is preferred over dopamine in heart failure?

It does not generate the same degree of vasoconstriction that is created by dopamine. Unlike dopamine, dobutamine does not have any effect on the α2‐adrenergic receptors. Dobutamine is preferred when there is a need to improve low cardiac output.

Inotropes Explained Clearly | Digoxin | Dobutamine | Milrinone

Why would you use dopamine over dobutamine?

Dobutamine produced a distinct increase in cardiac index, while lowering left ventricular end-diastolic pressure and leaving mean aortic pressure unchanged. Dopamine also significantly improved cardiac index, but at the expense of a greater increase in heart rate than occurred with dobutamine.

Is milrinone better than dobutamine?

Milrinone was introduced as an agent which, compared to dobutamine, cause reduced left and right heart filling pressures due to its greater reduction in vascular resistance, and thus superior to dobutamine in treatment of low cardiac output syndrome following cardiac surgery [30, 31].

What does milrinone do to the heart?

Milrinone is a vasodilator that works by relaxing the muscles in your blood vessels to help them dilate (widen). This lowers blood pressure and allows blood to flow more easily through your veins and arteries. Milrinone is used as a short-term treatment for life-threatening heart failure.

What are the indications for milrinone?

Milrinone Injection is indicated for the short-term treatment of severe congestive heart failure unresponsive to conventional maintenance therapy, and for the treatment of patients with acute heart failure, including low output states following cardiac surgery.

What is dobutamine used to treat?

Dobutamine is indicated for patients who require positive inotropic support in the treatment of cardiac decompensation due to depressed contractility resulting either from organic heart disease or from cardiac surgical procedures, especially when a low cardiac output is associated with raised pulmonary capillary ...

How does dobutamine help heart failure?

Dobutamine is a cardiac inotrope useful in the acute treatment of congestive heart failure. Dobutamine improves cardiac output, decreases pulmonary wedge pressure, and decreases total systemic vascular resistance with little effect on heart rate or systemic arterial pressure.

How does milrinone help pulmonary hypertension?

Milrinone is an intravenously active phosphodiesterase inhibitor that acts rapidly and exerts both positive inotropic and direct vasodilator effects in patients with heart failure. The ability of milrinone to lower PVR in patients with heart failure has not been tested.

What effect does dobutamine have on heart rate?

Dobutamine may cause a marked increase in heart rate or blood pressure, especially systolic pressure. Approximately 10% of patients in clinical studies have had rate increases of 30 beats/minute or more, and about 7.5% have had a 50 mm Hg or greater increase in systolic pressure.

Does milrinone increase heart rate?

Milrinone increases cardiac index with reductions in arterial pressure, left ventricular end-diastolic pressure, and PVR. Heart rate can increase, although this is not a consistent response and bradycardia can also occur.

Does milrinone stimulate beta receptors?

classic inodilators (milrinone, dobutamine)

Dobutamine stimulates mostly beta-receptors, with very little stimulation of alpha-receptors. Milrinone inhibits intracellular adenylyl cyclase, thereby increasing intracellular cyclic AMP levels.

What is the mechanism of action of milrinone?

It functions by improving cardiac contractility (inotropy), cardiac relaxation (lusitropy), and inducing vasodilation and has the overall effect of increased cardiac output, improvement of left ventricle-arterial coupling, and enhanced cardiac mechanical efficiency.

What receptors does dobutamine work?

Dobutamine is a synthetic catecholamine that acts on alpha-1, beta-1 and beta-2 adrenergic receptors. In the heart, the stimulation of these receptors produces a relatively strong, additive inotropic effect and a relatively weak chronotropic effect.

What is amiodarone used to treat?

Amiodarone is a potent antiarrhythmic agent that is used to treat ventricular arrhythmias and atrial fibrillation. The drug prevents the recurrence of life-threatening ventricular arrhythmias and produces a modest reduction of sudden deaths in high-risk patients.

What is milrinone contraindicated for?

Milrinone is contraindicated in patients with a milrinone hypersensitivity. Solutions containing dextrose may be contraindicated in those with a known corn hypersensitivity. Do not use milrinone in patients with severe obstructive aortic or pulmonic valvular heart disease in lieu of surgical relief of the obstruction.

Does milrinone cause Vtach?

Holter recordings demonstrated that in some patients injection of PRIMACOR increased ventricular ectopy, including nonsustained ventricular tachycardia.

How does milrinone increase cardiac output?

At lower filling pressures, milrinone increases cardiac output more markedly than equally hypotensive doses of pure vasodilators. This response is accompanied by an increased left ventricular dP/dtmax and a shift in the left ventricular performance is associated with a lower myocardial energy requirement.

Is milrinone a positive Inotrope?

Milrinone is a potent positive inotropic and vascular smooth muscle-relaxing agent in vitro, and therefore, it is not known to what extent each of these actions contributes to the drug's hemodynamic effects in patients with heart failure.

Do Positive inotropes increase heart rate?

Positive inotropes help the heart pump more blood with fewer heartbeats. This means that although the heart beats less, it also beats with more force to meet the oxygen demands of your body.

Can milrinone and dobutamine be given together?

The combined use of both I.V. milrinone and dobutamine has been used clinically and may have the advantage of improving hemodynamics better than either drug alone.

Is dobutamine a positive Inotrope?

Dobutamine is a more effective positive inotrope than dopamine with less chronotropic effects, although it does not dilate the renal vascular bed. Its plasma half-life is ~2 min. Dobutamine is prepared as a solution to be diluted with 5% dextrose or normal saline.
