Electrical axis between –90° and –180° strongly suggests ventricular tachycardia (although antidromic AVRT is a differential diagnosis). If the electrical axis during tachycardia differs >40° from the electrical axis during sinus rhythm, it also suggests ventricular tachycardia.

How do you Recognise ventricular tachycardia?

1. Fast heartbeat or a fluttering feeling in the chest (palpitations)
2. Dizziness.
3. Lightheadedness.
4. Chest pain.
5. Neck tightness.
6. Shortness of breath.
7. Fainting.
8. Cardiac arrest.

What does V tach look like on EKG?

Ventricular tachycardia refers to a wide QRS complex heart rhythm — that is, a QRS duration beyond 120 milliseconds — originating in the ventricles at a rate of greater than 100 beats per minute.

Can V tach have P waves?

The rate of V. Tach is from about 100-250 bpm. P Waves may be present or absent. P Waves are usually not seen if the rate is increased.

Which type of arrhythmia is associated with the absence of P waves on an electrocardiogram?

A junctional rhythm is characterized by QRS complexes of morphology identical to that of sinus rhythm without preceding P waves.

Which electrocardiogram ECG characteristic in a patient with acute coronary syndrome suggests myocardial ischemia?

ECG Signs of Myocardial Infarction

ECG changes of infarction include ST elevation (indicating injury), Q waves (indicating necrosis), and T-wave inversion (indicating ischemia and evolution of the infarction).

What ECG has no P wave?

However, if there are no P waves and the QRS complexes appear at randomly irregular intervals, the diagnosis is atrial fibrillation.

How can you differentiate ventricular tachycardia from supraventricular tachycardia?

During SVT, the tachycardia originates from the atria or involves the atria in the tachycardia circuit. During VT, cardiac activation originates from the ventricle and atrial activation may or may not be linked to ventricular activation.

How can you distinguish ventricular tachycardia from ventricular fibrillation?

Ventricular tachycardia (v-tach is treated similarly to v-fib. The difference is that ventricular tachycardia continues to make the heartbeat regularly, but it goes so fast that the heart never gets a chance to fill with blood. 5 There's not an opportunity to build up the pressure, so the blood stops flowing.

How does ventricular tachycardia occur?

What is ventricular tachycardia? Ventricular tachycardia (VT or V-tach) is a type of abnormal heart rhythm, or arrhythmia. It occurs when the lower chamber of the heart beats too fast to pump well and the body doesn't receive enough oxygenated blood.

How is ventricular tachycardia diagnosed on ECG?

1. The initial portion of the QRS complex is smooth in ventricular tachycardia. ...
2. R-wave duration ≥40 ms suggest ventricular tachycardia.
3. Duration from start of QRS complex to nadir of S-wave ≥60 ms suggests ventricular tachycardia.

Do you have a pulse with ventricular tachycardia?

V-tach occurs when your pulse rate is more than 100 beats per minute, and you have at least three irregular heartbeats, or arrhythmias, in a row. Besides palpitations, V-tach can cause symptoms like: Chest pain.

Is ventricular tachycardia regular or irregular?

Sustained monomorphic ventricular tachycardia is usually regular; that is, it is associated with constant R-R intervals. In several cases, however, the cycles of ventricular tachycardia are more or less variable.

What is the difference between monomorphic and polymorphic ventricular tachycardia?

Monomorphic ventricular tachycardia is a more organized rhythm than the polymorphic form, and patients may maintain a reasonable hemodynamic state. In the absence of hypotension, monomorphic ventricular tachycardia can be treated with intravenous sotalol (1 mg/kg to a maximum of 100 mg) or amiodarone (5 mg/kg).

What does monomorphic ventricular tachycardia look like?

Very broad QRS complexes (~ 200 ms) with uniform morphology. Fusion and capture beats are seen in the rhythm strip. Brugada's sign is present: the time from the onset of the QRS complex to nadir of S wave is > 100 ms (best seen in V6).

Which waveform characteristic is seen with a conduction delay?

A wider QRS indicates that conduction is occurring through an abnormal pathway. The wider QRS is indicative of the delay in conduction as it takes longer for electrical activity to travel from myocyte to myocyte compared to the electrical conducting system.

How is ventricular fibrillation defined?

An arrhythmia that starts in your ventricle is called ventricular fibrillation. This occurs when the electrical signals that tell your heart muscle to pump cause your ventricles to quiver (fibrillate) instead. The quivering means that your heart is not pumping blood out to your body.

What is ventricular fibrillation and tachycardia?

A dangerous condition related to ventricular tachycardia is ventricular fibrillation (V-fib). In V-fib , your lower heart chambers contract in a very rapid and uncoordinated manner. This abnormal rhythm happens most often in people with heart disease or a prior heart attack.

What is ECG Aberrancy?

Aberrant ventricular conduction is a common electrocardiographic (EKG) manifestation that occurs when the supraventricular electrical impulse is conducted abnormally through the ventricular conducting system. This results in a wide QRS complex that may be confused with a ventricular ectopic beat.

What part of the ECG would you look at to determine if an arrhythmia is supraventricular?

The QRS complexes of ventricular arrhythmias are typically wide and bizarre. The QRS complexes may also be wide and bizarre in supraventricular arrhythmia – one that originates in the SA node or an ectopic pacemaker in the atria or AV junction – if a bundle branch block or aberrant ventricular conduction is present.

What is the S wave ECG?

The S wave is the first downward deflection of the QRS complex that occurs after the R wave. However, a S wave may not be present in all ECG leads in a given patient. Enlarge. In the normal ECG, there is a large S wave in V1 that progressively becomes smaller, to the point that almost no S wave is present in V6.

What is AP wave ECG?

Definition/Introduction. The P wave and PR segment is an integral part of an electrocardiogram (ECG). It represents the electrical depolarization of the atria of the heart. It is typically a small positive deflection from the isoelectric baseline that occurs just before the QRS complex.

What is monomorphic ventricular tachycardia?

Sustained monomorphic ventricular tachycardia (SMVT) is defined by the following characteristics: ●A regular wide QRS complex (≥120 milliseconds) tachycardia at a rate greater than 100 beats per minute. ●The consecutive beats have a uniform and stable QRS morphology.

Which ECG finding is most consistent with a myocardial infarction?

One of the most significant findings of myocardial infarction is the presence of ST segment elevation. The ST segment is the part of the ECG tracing that starts at the end of the S wave and ends at the beginning of the T wave. The point where the end of the Q wave and the ST segment meet is called the J point.

Which characteristic on the ECG typically indicates myocardial ischemia quizlet?

The amplitude of acute ischemic ST elevation indicates the severity of the ischemia. If marked ST elevation or depression in multiple leads is observed, consider severe ischemia or ischemia affecting large regions of the myocardium.
