Epiploic appendages are peritoneal structures that arise from the outer serosal surface of the bowel wall towards the peritoneal pouch. They are filled with adipose tissue and contain a vascular stalk.

What causes epiploic?

Primary epiploic appendagitis is caused by torsion or spontaneous venous thrombosis of the involved epiploic appendage. Secondary epiploic appendagitis is associated with inflammation of adjacent organs, such as diverticulitis, appendicitis or cholecystitis.

What is epiploic?

Epiploic appendagitis refers to the inflammation of small pouches on the colon or large intestine. These pouches are called epiploic appendages. These pouches help the body absorb nutrients, and they protect blood vessels in the large intestine. Most people have about 50–100 epiploic appendices.

Where are epiploic appendages found?

Epiploic appendages are small outpouchings of fat-filled, serosa-covered structures present on the external surface of the colon projecting into the peritoneal cavity. Each appendage encloses small branches of the circular artery and vein that supply the corresponding segment of the colon.

Why does epiploic appendagitis happen?

Primary epiploic appendagitis occurs when the blood supply to your epiploic appendages gets cut off. Sometimes an appendage gets twisted, which pinches blood vessels and stops the flow of blood. In other cases, the blood vessels can suddenly collapse or get a blood clot. This blocks the flow of blood to the appendage.

Epiploic appendagitis: what the heck is that?

How is epiploic appendagitis diagnosis?

Epiploic appendagitis is usually diagnosed with CT imaging, and the classic CT findings include: (i) fat-density ovoid lesion (hyperattenuating ring sign), (ii) mild bowel wall thickening, and (iii) a central high-attenuation focus within the fatty lesion (central dot sign).

Can epiploic appendagitis come back?

Epiploic appendagitis (EA) is a rare cause of acute abdominal pain that is usually benign and self-limiting and can be treated conservatively with analgesics and antiinflammatory medications (1–3). Recurrence of EA is rare, and documented cases describe abdominal pain recurring at the same location (3–5).

Is epiploic appendagitis rare?

It's relatively rare and has only recently become more easily noticed in the medical world. Because it's not well known, it's often misdiagnosed and confused with other conditions that also cause severe stomach pain. Epiploic appendagitis is relatively understudied, and research is in early stages.

Can a hernia cause epiploic appendagitis?

Epiploic appendagitis within a concomitant femoral hernia is rare but a consideration when a patient attends with a painful lump in the groin. CT appearances are diagnostic.

Symptoms include acute and recurrent non-migratory abdominal pain, local tenderness, postprandial fullness, early satiety, epigastric discomfort, vomiting, bloating, diarrhea, intermittent febrile temperature, and moderate weight loss.

What is a fatty colon?

Lipomas of the colon are a benign growth of adipose tissue found in the submucosa. They are often incidentally detected during imaging, colonoscopy, surgery or autopsy. The first colonic lipoma was reported by Bauer in 1757 [1].

Why do diverticula cause sudden pain?

Sometimes, though, the little pockets in your colon can become inflamed or infected. When these pockets become infected, it can cause a flare-up or attack known as diverticulitis. Until it's treated or the inflammation eases, diverticulitis can cause sharp pain, along with other symptoms.

Does epiploic appendagitis cause diarrhea?

The typical clinical manifestation of epiploic appendagitis is abdominal pain that is acute, moderate to severe, colicky, and continuous over the corresponding area of the colon. Sometimes there is vomiting; however, fever and diarrhea are unusual.

Do you need surgery for epiploic appendagitis?

Conclusion: Epiploic appendagitis can be managed safely by surgical resection. As described by Sand et al, there are more common diseases with similar symptoms. Therefore, surgery may be a necessary diagnostic and therapeutic tool for this pathology.

What is calcified epiploic appendage?

The epiploic appendages of the colon have long been an anatomical curiosity. In 1863, Virchow (1) first described the formation of foreign bodies in the abdomen from these calcified appendages. In 1924, Klingenstein (2) noted that 12 cases of such foreign bodies had been described in the literature.

Why appendix is called vermiform appendix?

What does Pericolonic fat stranding mean?

In conclusion, pericolic fat stranding identified on CT images of colon cancer is demonstrated more frequently with increasing circumferential proportion of the tumor and longitudinal length. The presence of pericolic fat stranding suggests tumor invasion of T3 or more, but cannot distinguish between T3 and T4.

What does diverticulitis poop look like?

The most common presentations of symptomatic patients with lipomas greater than 2 cm in size include abdominal pain, hemorrhage, diarrhea, or constipation. Lipomas that grow more than 4 cm in size can lead to obstruction and intussusception requiring surgical or endoscopic resection.

Can a large mass in colon be benign?

Giant lipomas (>4 cm) are the most common benign tumors in the colon that cause intussusception, although no specific incidence data have been documented. Even patients with large lipomas may have nonspecific or intermittent symptoms, which causes delay and difficulty in making the diagnosis.

What is a submucosal lipoma?

Colonic submucosal lipomas are rare benign tumours of the colon, which may be discovered incidentally at colonoscopy, through imaging such as CT or at autopsy. These tumours can cause complications such as bleeding, intussusception and bowel obstruction.

Can a lipoma become cancerous?

Cancerous tumours of the fat cells are called liposarcomas. They are a type of soft tissue sarcoma. It is very rare for lipomas to turn into a cancerous sarcoma. It is still important to tell your doctor if your lipoma changes in any way or if you get any new lumps.
