The thiazolidinediones (or 'glitazones') are a new class of drugs for the treatment of type 2 diabetes. They bind avidly to peroxisome proliferator-activated receptor gamma in adipocytes to promote adipogenesis and fatty acid uptake (in peripheral but not visceral fat).

What is the mode of action of thiazolidinediones?

TZDs exert their antidiabetic effects through a mechanism that involves activation of the gamma isoform of the peroxisome proliferator-activated receptor (PPAR gamma), a nuclear receptor.

How do TZDs work?

How do glitazones work? TZDs work by targeting the PPAR-gamma receptor, which activates a number of genes in the body and plays an important role in how the body metabolises glucose and how the body stores fat.

How do Thiazolidinediones lower blood sugar?

TZDs help keep your blood glucose levels on target by decreasing insulin resistance and making body tissues more sensitive to insulin's effects. Then glucose can enter your cells where it is needed. TZDs also cut down on the amount of glucose made by your liver, which can be far too much in people with type 2 diabetes.

What are thiazolidinediones for?

Metformin and thiazolidinediones (TZDs) are used for the treatment of type 2 diabetes. As both drugs ameliorate hyperglycemia by improving glucose metabolism of target tissues of insulin, they are particularly prescribed to patients with severe insulin resistance.

How does a DPP-4 inhibitor work?

DPP-4 inhibitors work by blocking the action of DPP-4, an enzyme which destroys the hormone incretin. Incretins help the body produce more insulin only when it is needed and reduce the amount of glucose being produced by the liver when it is not needed.

What is an example of a thiazolidinediones?

Oral tablet – There are two types of thiazolidinediones that are currently available: Pioglitazone (marketed as Actos) Rosiglitazone (marketed as Avandia). Combination pills containing pioglitazone and rosiglitazone along with other diabetes medications such as metformin are available.

How does thiazolidinediones work in diabetes?

The thiazolidinediones (or 'glitazones') are a new class of drugs for the treatment of type 2 diabetes. They bind avidly to peroxisome proliferator-activated receptor gamma in adipocytes to promote adipogenesis and fatty acid uptake (in peripheral but not visceral fat).

When do you use TZD?

Use of TZDs in Patients With Diabetes and Symptomatic Heart Disease. In patients with class I or II NYHA CHF categories, TZDs may be used cautiously, with initiation of treatment at the lower dosage of each drug (e.g., rosiglitazone 2 mg q.d. or pioglitazone 15 mg q.d.).

How long does it take for Tzd to work?

Thiazolidinediones lower hemoglobin A_Ic levels by as much as 1.0% to 1.5%. Effects can be seen in as little as 4 weeks, but full lowering takes 6 to 12 weeks. When used in combination with other diabetic agents, such as sulfonylureas and biguanides, TZDs' hypoglycemic effects appear to be complementary.

What is the side effects of thiazolidinediones?

Conclusions: Common side effects associated with TZDs include edema, weight gain, macular edema and heart failure. Moreover, they may cause hypoglycemia when combined with other antidiabetic drugs as well as decrease hematocrit and hemoglobin levels. Increased bone fracture risk is another TZD-related side effect.

How do GLP 1 receptor agonists work?

The GLP-1RAs have been shown to significantly improve glycemic parameters and reduce body weight. These agents work by activating GLP-1 receptors in the pancreas, which leads to enhanced insulin release and reduced glucagon release-responses that are both glucose-dependent-with a consequent low risk for hypoglycemia.

Why was rosiglitazone withdrawn?

Rosiglitazone was withdrawn from the New Zealand market April 2011 because Medsafe concluded the suspected cardiovascular risks of the medicine for patients with type 2 diabetes outweigh its benefits.

Do thiazolidinediones increase insulin production?

Thiazolidinediones, when used as monotherapy, are not associated with hypoglycemia, as they do not increase insulin secretion. Thiazolidinediones decrease the secretory demands and the strain on the β-cell caused by chronic insulin resistance and may preserve pancreatic β-cell function.

What is the mechanism of action of exenatide?

Mechanism of Action

Exenatide is a GLP-1 receptor agonist released from the gut and acts to increase glucose-dependent insulin secretion from pancreatic beta cells, suppress glucagon secretion, delay gastric emptying, and reduce food intake. The binding of the drug to pancreatic GLP-1 receptors mediates these actions.

What is the mechanism of action of rosiglitazone?

Mechanism of Action: Rosiglitazone, a member of the thiazolidinedione class of antidiabetic agents, improves glycemic control by improving insulin sensitivity. Rosiglitazone is a highly selective and potent agonist for the peroxisome proliferator-activated receptor-gamma (PPARγ).

Why does TZD cause weight gain?

TZD-associated weight gain may result mainly from increased fat mass and fluid retention and may be in part congruent to the mechanism of action of TZD. Increases in fat mass are almost exclusively limited to subcutaneous fat, while there are no effects or even decreases in visceral fat.

Who should not take thiazolidinediones?

[24] High risk of fractures: Due to increased fracture risk, patients who are at a high risk of fractures, such as those with a history of osteoporosis, postmenopausal women, or patients taking other medications that increase fracture risk (such as glucocorticoids and PPIs), should not start on TZD therapy.

How do SGLT2 inhibitors work?

SGLT2 inhibitors work by preventing the kidneys from reabsorbing glucose back into the blood. This allows the kidneys to lower blood glucose levels and the excess glucose in the blood is removed from the body via urine.

What is the mechanism of action of sulfonylureas?

These drugs exert their hypoglycaemic effects by stimulating insulin secretion from the pancreatic beta-cell. Their primary mechanism of action is to close ATP-sensitive K-channels in the beta-cell plasma membrane, and so initiate a chain of events which results in insulin release.

How are sulphonylureas administered?

How are Sulphonylureas taken? It is taken as a tablet once or twice per day either just before or shortly before eating. The dose you are prescribed may be altered by your doctor or nurse depending on your blood glucose levels.

What are antidiabetic drugs used for?

Antidiabetic drugs are medicines developed to stabilise and control blood glucose levels amongst people with diabetes. Antidiabetic drugs are commonly used to manage diabetes. There are a number of different types of antidiabetic drug including: Insulin.

What drugs are SGLT2?

SGLT2 inhibitors are a class of prescription medicines that are FDA-approved for use with diet and exercise to lower blood sugar in adults with type 2 diabetes. Medicines in the SGLT2 inhibitor class include canagliflozin, dapagliflozin, and empagliflozin.

How does sitagliptin work in the body?

Sitagliptin works by increasing the amount of insulin that your body makes. Insulin is the hormone that controls sugar levels in your blood. You take sitagliptin once a day. The most common side effect of sitagliptin is headaches.

What is the function of incretin?

Incretins are gut hormones that are secreted from enteroendocrine cells into the blood within minutes after eating. One of their many physiological roles is to regulate the amount of insulin that is secreted after eating.
