Cor pulmonale is a condition that causes the right side of the heart to fail. Long-term high blood pressure in the arteries of the lung and right ventricle of the heart can lead to cor pulmonale.

What does cor pulmonale stand for?

: disease of the heart characterized by hypertrophy and dilatation of the right ventricle and secondary to disease of the lungs or their blood vessels.

How does cor pulmonale affect the lungs?

Acute cor pulmonale

This occurs when part of a blood clot breaks away and travels to the lungs through the bloodstream. The clot can cause a blockage and damage the lungs. Doctors may be able to treat a small clot, but a large clot can stop blood from reaching the lungs and be fatal .

What are the key features of cor pulmonale?

Cor pulmonale itself is usually asymptomatic but common physical findings include a left parasternal systolic lift, a loud pulmonic component of S2, functional tricuspid and pulmonic insufficiency murmurs, and later, distended jugular veins, hepatomegaly, and lower-extremity edema.

Is cor pulmonale the same as congestive heart failure?

What is cor pulmonale? Cor pulmonale is a condition that most commonly arises out of complications from high blood pressure in the pulmonary arteries (pulmonary hypertension). It's also known as right-sided heart failure because it occurs within the right ventricle of your heart.

Cor pulmonale - causes, symptoms, diagnosis, treatment, pathology

What are the signs and symptoms of cor pulmonale?

• Fainting spells during activity.
• Chest discomfort, usually in the front of the chest.
• Chest pain.
• Swelling of the feet or ankles.
• Symptoms of lung disorders, such as wheezing or coughing or phlegm production.
• Bluish lips and fingers (cyanosis)

How might you Recognise cor pulmonale on an ECG?

This ECG shows some typical abnormalities that may be seen in cor pulmonale and other chronic pulmonary diseases: (1) R/S ratio >1 in V1 and <1 in V6 suggestive of right ventricular hypertrophy/enlargement, (2) right superior axis deviation, (3) left atrial type of p wave with increased width of the p wave and biphasic ...

What is cor pulmonale pathophysiology?

The pathophysiology of cor pulmonale is a result of increased right-sided filling pressures from pulmonary hypertension that is associated with diseases of the lung.

What are the complications of cor pulmonale?

Complications of cor pulmonale include syncope, hypoxia, pedal edema, passive hepatic congestion, and death.

What is cor pulmonale NHS?

Description. Cor pulmonale describes impairment in right ventricular function as a result of respiratory disease, leading to increased resistance to blood flow in the pulmonary circulation.

How do you manage cor pulmonale?

Treatment strategies for cor pulmonale include supplemental oxygen, assisted mechanical ventilation, digoxin, and diuretics. Pulmonary vasodilator compounds should be used with caution because they can compromise gas exchange in cor pulmonale from secondary pulmonary hypertension.

Does cor pulmonale cause chest pain?

It is the most common cause of cor pulmonale. It is often caused by smoking or being exposed to smoky or poorly ventilated environments. Its symptoms include wheezing, chest pain, trouble breathing, respiratory infections, lethargy, weight loss, and swelling of the lower limbs.

How can cor pulmonale be treated with COPD?

Medical therapy for chronic cor pulmonale is generally focused on treatment of the underlying pulmonary disease and improving oxygenation and right ventricular (RV) function by increasing RV contractility and decreasing pulmonary vasoconstriction.

What is cor pulmonale COPD?

Cor pulmonale was classically defined as “hypertrophy of the right ventricle resulting from diseases affecting the function and/or structure of the lungs except when these pulmonary alterations are the result of diseases that primarily affect the left side of the heart” (^{WHOexpertcommitteereport1963}).

What is acute pulmonary embolism with acute cor pulmonale?

Acute pulmonary embolism (PE) is the prototype for acute cor pulmonale. Acute obstruction of the pulmonary vasculature may lead to acute right-sided heart failure, and at times, total cardiovascular collapse.

What is thrombosis in the lungs?

Pulmonary embolism is a blockage in one of the pulmonary arteries in your lungs. In most cases, pulmonary embolism is caused by blood clots that travel to the lungs from deep veins in the legs or, rarely, from veins in other parts of the body (deep vein thrombosis).

Can high blood pressure cause coughing up blood?

Bleeding in the lungs.

Pulmonary hypertension can lead to life-threatening bleeding into the lungs and coughing up blood (hemoptysis).

Why does right sided heart failure occur?

High blood pressure in the pulmonary arteries increases the workload of the right ventricle. Over time, this causes the right ventricle to fail. This is blockage of the arteries that supply blood to your heart. CAD can cause left-sided heart failure leading to right-sided heart failure.

Can cor pulmonale cause pulmonary edema?

In chronic cor pulmonale with right ventricular hypertrophy and pulmonary hypertension, acute pulmonary edema may de- velop. This is particularly true in the forms causing no destruction of capillaries and no pulmonary ischemia. which, in itself, predisposes to edema.

What causes acute cor pulmonale?

Massive pulmonary embolism is the most common cause of acute cor pulmonale. A massive pulmonary embolism can mimic a myocardial infarction with elevated troponins, ST changes, chest pain, and shortness of breath. Chronic obstructive pulmonary disease (COPD) is the most common cause of cor pulmonale.

What is the ICD 10 code for cor pulmonale?

I27. 81 is a billable/specific ICD-10-CM code that can be used to indicate a diagnosis for reimbursement purposes.

What is decompensated cor pulmonale?

Decompensated cor pulmonale is a clinical syndrome manifested by signs of congestive heart failure in pulmonary disease.

What is S1Q3T3?

Discussion: The McGinn-White sign or, more commonly known as S1Q3T3 pattern, is a nonspecific finding associated with right heart strain1. A common misconception is the sole association of this sign with a pulmonary embolism, which is just one possible etiology of right heart strain.

Can an EKG diagnose a pulmonary embolism?

2 The ECG is often abnormal in PE, but findings are neither sensitive nor specific for the diagnosis of PE.

Does echocardiogram show pulmonary embolism?

Transesophageal echocardiography may identify central pulmonary embolism, and the sensitivity for central pulmonary embolism is reported to be 82%. Overall sensitivity and specificity for central and peripheral pulmonary embolism is 59% and 77%. Echocardiography (ECHO) provides useful information.
