Isosorbide dinitrate belongs to the group of medicines called nitrates. It works by relaxing the blood vessels and increasing the supply of blood and oxygen to the heart while reducing its work load. When used regularly on a long-term basis, this helps prevent angina attacks from occurring.

What is the mechanism of action of isosorbide dinitrate?

The principal pharmacological action of isosorbide dinitrate is relaxation of vascular smooth muscle and consequent dilatation of peripheral arteries and veins, especially the latter.

What does isosorbide dinitrate do?

ISOSORBIDE DINITRATE (eye soe SOR bide dye NYE trate) is a type of vasodilator. It relaxes blood vessels, increasing the blood and oxygen supply to your heart. This medicine is used to prevent chest pain caused by angina. It will not help to stop an episode of chest pain.

How does isosorbide affect blood pressure?

There were no side effects, including hypotension, orthostatic effect, and reflex tachycardia. Sublingual isosorbide safely and effectively lowers systolic and diastolic BP in patients with severe, uncontrolled arterial hypertension.

Is isosorbide dinitrate short or long acting?

Long-acting nitrate preparations

All the nitrates (GTN, isosorbide dinitrate, and isosorbide mononitrate) come in long-acting preparations.

How Do Nitrates Work? (Nitroglycerin)

Do nitrates reduce preload or afterload?

Organic nitrate esters have a direct relaxant effect on vascular smooth muscles, and the dilation of coronary vessels improves oxygen supply to the myocardium. The dilation of peripheral veins, and in higher doses peripheral arteries, reduces preload and afterload, and thereby lowers myocardial oxygen consumption.

How does Nitro decrease preload?

NTG reduces preload via venous dilation, and achieves modest afterload reduction via arterial dilation. These effects result in decreased myocardial oxygen demand. In addition, NTG induces coronary vasodilation, thereby increasing oxygen delivery.

Can isosorbide dinitrate cause low blood pressure?

Taking isosorbide dinitrate with certain other medicines can cause severely low blood pressure. This includes medicine to treat erectile dysfunction or pulmonary arterial hypertension. Serious, life-threatening side effects may occur.

What is the difference between isosorbide mononitrate and dinitrate?

Isosorbide mononitrate: Metabolized principally in the liver, but unlike isosorbide dinitrate, does not undergo first-pass metabolism; metabolites appear to be pharmacologically inactive.

Is isosorbide a diuretic?

Use isosorbide solution (diuretic) as ordered by your doctor.

Is isosorbide dinitrate a vasodilator?

Hydralazine and isosorbide dinitrate are each vasodilator drugs (153). The combination of these two drugs is used for treating heart disease.

When do you use isosorbide dinitrate?

To prevent angina attacks, isosorbide dinitrate is usually taken at regular intervals. To treat an angina attack that has already begun, use the medicine at the first sign of chest pain. Place the tablet under your tongue and allow it to dissolve slowly. Do not chew or swallow it.

What is adverse effect of isosorbide dinitrate?

Headache, which may be severe, is the most commonly reported side effect. Headache may be recurrent with each daily dose, especially at higher doses. Transient episodes of lightheadedness, occasionally related to blood pressure changes, may also occur.

What is the mechanism by which isosorbide dinitrate increases cyclic GMP levels?

Arteriolar relaxation reduces systemic vascular resistance, systolic arterial pressure, and mean arterial pressure. Isosorbide dinitrate is converted to the active nitric oxide to activate guanylate cyclase. This activation increases levels of cyclic guanosine 3',5'-monophosphate (cGMP).

Is isosorbide dinitrate a calcium channel blocker?

Imdur (isosorbide mononitrate) Extended Release and Norvasc (amlodipine) are indicated for the prevention of angina pectoris. Norvasc is also prescribed for the treatment of high blood pressure. Imdur and Norvasc belong to different drug classes. Imdur is a vasodilator and Norvasc is a calcium channel blocker (CCB).

How do you use isosorbide dinitrate sublingual?

Place 1 tablet under the tongue and allow it to dissolve as directed by your doctor. Do not chew or swallow the tablet. The dosage is based on your medical condition and response to treatment. If you are using this medication to prevent chest pain before physical activities, use it 15 minutes before the activity.

What is trimetazidine 35mg?

Trimetazidine is an anti-ischemic metabolic modulator [120], with similar anti-anginal efficacy to propranolol in doses of 20mg thrice daily. From: Coronary Artery Disease, 2018.

Is isosorbide same as Isordil?

While both isosorbide dinitrate (Isordil) and nitroglycerin are known as nitrate vasodilators, they are not the same medication. Isosorbide dinitrate (Isordil) works slowly to help prevent chest pain.

Is imdur and isosorbide the same?

Are Imdur and Isordil the Same Thing? Imdur (isosorbide mononitrate) Extended Release and Isordil (isosorbide dinitrate) are vasodilators indicated for the prevention of angina pectoris due to coronary artery disease. Side effects of Imdur and Isordil that are similar include headache and dizziness.

What is the best time of day to take isosorbide?

You should take this medicine first thing in the morning and follow the same schedule each day. This medicine works best if you have a "drug-free" period of time every day when you do not take it. Your doctor will schedule your doses during the day to allow for a drug-free time.

Is it OK to take isosorbide at night?

Not all brands and forms of isosorbide mononitrate are taken the same number of times per day. You may need to take the medicine only once daily, in the morning after getting out of bed.

Is isosorbide dinitrate crushable?

Swallow the extended-release tablet or capsule whole. Do not split, crush, or chew it.

How do you know if nitroglycerin is working?

Do not chew, crush, or swallow nitroglycerin sublingual tablets. Instead, place the tablet under your tongue or between your cheek and gum and wait for it to dissolve. You may feel burning or tingling in your mouth as the tablet dissolves. This is normal but is not a sign that the tablet is working.

Is nitroglycerin a diuretic?

We advocate adding intravenous (IV) nitroglycerin (NTG), as a potent vasodilator, to diuretics to provide rapid alleviation of acute shortness of breath and to simultaneously increase cardiac output while decreasing preload and afterload.

Does nitroglycerin work on arteries or veins?

Although nitroglycerin has a vasodilatory effect in both arteries and veins, the profound desired effects caused by nitroglycerin are primarily due to venodilation.
