Thrombin is an agent that causes clotting. It may be injected under ultrasound guidance into the pseudoaneurysm to clot the blood inside it. The clot is gradually reabsorbed. The procedure is performed under local anaesthetic.

What is a thrombin injection?

Ultrasound-guided thrombin injection of femoral pseudoaneurysms is safe, quick, painless, and effective. It is superior to compression therapy (less painful with a higher success rate) and surgery. Ultrasound-guided thrombin injection should be the initial treatment of femoral pseudoaneurysms.

Can topical thrombin be injected?

In late 2016, CHPSO, one of the largest federally designated patient safety organizations, created in 2008 by the California Hospital Association, published a case report about accidental intravenous (IV) administration of RECOTHROM (recombinant topical thrombin).

How is femoral pseudoaneurysm treated?

Management options include observation, ultrasound-guided compression, ultrasound-guided thrombin injection, or surgery. For small, stable pseudoaneurysms (<2 cm), observation with weekly duplex ultrasound until thrombosis occurs is appropriate. Anticoagulation should be held; patients should avoid lifting or bending.

What is false aneurysm?

A pseudoaneurysm, or pseudoaneurysm of the vessels, occurs when a blood vessel wall is injured and the leaking blood collects in the surrounding tissue. It is sometimes called a false aneurysm. In a true aneurysm, the artery or vessel weakens and bulges, sometimes forming a blood-filled sac.

What are the 3 types of aneurysms?

The three types of cerebral aneurysms are: berry (saccular), fusiform and mycotic. The most common, "berry aneurysm," occurs more often in adults.

Can a pseudoaneurysm heal itself?

Some pseudoaneurysms resolve themselves, though others require treatment to prevent hemorrhage, an uncontrolled leak or other complications. An ultrasound study in the Vascular Laboratory may be requested to evaluate a puncture site if swelling, pain or extensive bruising suggests a pseudoaneurysm may have developed.

How long does it take for pseudoaneurysm to heal?

A small pseudoaneurysm may close on its own in about 4 weeks. You may need any of the following to treat a pseudoaneurysm that does not close: Debridement is a procedure used to remove dead tissue. You may need this if the area around your pseudoaneurysm becomes infected.

How long does it take for femoral artery to heal?

It may take 6 to 8 weeks to fully recover. Follow-up care is a key part of your treatment and safety. Be sure to make and go to all appointments, and call your doctor if you are having problems.

How can pseudoaneurysm be reduced?

Currently the treatment options for pseudoaneurysms include ultrasound guided compression (USGC), thrombin therapy, arterial embolisation, endovascular stent graft insertion and surgery.

What is the route of administration of thrombin?

Thrombin is not FDA-approved for percutaneous administration. Thrombin has been administered intravascularly as a solution directly into pseudoaneurysms under ultrasound guidance via a percutaneous injection. [32332] [32334] [32353] NOTE: Thrombin is not FDA-approved for the treatment of pseudoaneurysms.

What is the function of thrombin?

Thrombin is an unique molecule that functions both as a procoagulant and anticoagulant. In its procoagulant role it activates platelets through its receptor on the platelets. It regulates its own generation by activating coagulation factors V, VIII and even XI resulting in a burst of thrombin formation.

Is thrombin a drug?

What Is Thrombin Used For and How Does it Work? Thrombin is used to prevent and stop bleeding whenever oozing blood and minor bleeding from microvessels is accessible. Thrombin is available under the following different brand names: Recothrom, Thrombogen, and Thrombin JMI.

What's the meaning of thrombin?

: a proteolytic enzyme that is formed from prothrombin and facilitates the clotting of blood by catalyzing conversion of fibrinogen to fibrin.

Do leg stents work?

New Johns Hopkins research suggests that people who undergo minimally invasive placement of stents to open clogged leg arteries are significantly less likely than those who have conventional bypass surgery to need a second treatment for the condition within two years.

What is a femoral puncture?

The target for femoral puncture is the mid point of the common femoral artery (CFA), defined as the segment between the inferior epigastric artery, and the bifurcation of the superficial and profunda branches, and is usually at the level of the center of the femoral head [see below, Figure 6].

What happens when you get a stent in your leg?

Rarely, when a stent is being inserted, dangerous bleeding may happen and the stent may come apart. Both procedures can, in rare cases, cause a partly blocked artery to close up completely. Then you may need an emergency procedure. If the procedure cannot be done right away, your leg may have to be amputated.

Is a pseudoaneurysm life threatening?

Traumatic pseudoaneurysm of the facial artery is a rare complication after a routine procedure such as extraction of an impacted lower third molar. However, extreme careful management must be taken when the neurovascular bundle is exposed during complicated extractions as it can induce a life-threatening situation.

What happens when a pseudoaneurysm ruptures?

It occurs when the wall of a blood vessel is damaged. This can cause blood to leak out of the blood vessel and to collect in the surrounding tissue. If you have a pseudoaneurysm, it's important to get a diagnosis and the appropriate treatment because some pseudoaneurysms, if left untreated, may rupture.

What does a pseudoaneurysm feel like?

Symptoms of pseudoaneurysm include pain due to increased pressure from swelling or nerve compression, and extremity swelling due to venous compression. Further complications of pseudoaneurysms include deep venous thrombosis or rupture, the risk of which increases with increasing pseudoaneurysm size (1).

How painful is a pseudoaneurysm?

Pseudoaneurysms usually present as a painful, tender, pulsatile mass. The overlying skin is sometimes erythematous (red). They can sometimes be confused with abscesses. The patient may describe a history of catheterization or trauma, and they may note that the pulsatile mass is gradually expanding.

What is the difference between a pseudoaneurysm and a hematoma?

Altin, R, et al. Pseudoaneurysm and Arteriovenous Fistual After Femoral Artery Catheterization: Association with Low Femoral Puncture. AJR 1989. 152 (3):629-631.

Can you get a pseudoaneurysm in a vein?

Iatrogenic venous pseudoaneurysm following venipuncture is an extremely rare complication of a very common medical procedure. A review of the vascular surgical literature revealed that only two cases of venous pseudoaneurysm secondary to venipuncture have been reported in the past half-century.

Are there warning signs of an aneurysm?

An aneurysm happens when pressure causes a weak point in a blood vessel to balloon and form a small sac or bulge. Some warning signs include severe headache, nausea, vomiting, double vision, seizures, cardiac arrest, and loss of consciousness.

Can drugs cause aneurysm?

Mycotic aneurysms are caused by infections of the artery wall. Tumors and trauma can also cause aneurysms to form. Drug abuse, especially cocaine, can cause the artery walls to inflame and weaken. Brain aneurysms are a common occurrence.
